Affected information defines the class

Tift Regional Health System and Southwell data-settlement claims are due October 15, 2026 online or by mailing postmark. The class includes United States individuals whose information may have been compromised in the incident, including those sent breach notices.

The $1.2 million fund pays approved costs and awards before benefits. One cash option covers actual documented losses up to $5,000, with losses occurring between August 11, 2022 and October 15, 2026. The alternative is an estimated $75 proportional payment that can rise or fall.

Two cash choices do not stack

The FAQ offers one of the two cash choices along with two years of medical-identity monitoring. The monitoring insurance limit is not a cash award and the $75 option does not add to a maximum documented-loss reimbursement.

Use the incident notice, third-party records and actual claim form selections. Defendants deny the claims, and the court must approve the agreement before payment. A patient relationship alone does not establish class membership or guarantee that the displayed estimate will be received.
