Four locations and California residence limit this case

The ASI Foods settlement FAQ gives November 8, 2026 before 11:59 p.m. for cash claim forms. The case concerns California residents who paid a six-percent health-care surcharge at four San Francisco Domino’s locations between December 4, 2023 and August 12, 2026.

The listed locations are 3116 Noriega Street, 320 Bayshore Boulevard, 5200 Geary Boulevard and 728 Geary Street. The choices are a voucher worth 75 percent of the surcharge paid or a cash alternative worth 50 percent of that surcharge. Neither is 75 or 50 percent of the entire order price.

An automatic voucher is different from a cash claim

Class members who supplied a valid ordering email may receive a voucher without another step; choosing cash requires a claim. People without an email notice or with purchases not linked to that email need proof of purchase under the administrator’s instructions. Keep receipts rather than assume every Domino’s order nationwide qualifies.

Exclusions and objections are due October 24. The site says these defendants stopped passing on the fee in January 2026 and agreed to disclose it clearly if used again. They deny liability. The class dates and fee practices are separate facts; do not assume every purchase across the entire class period carried the charge.
