Class membership is broader than reimbursement eligibility

The EmblemHealth and New York City Senior Care copay settlement requires qualifying reimbursement claims by October 30, 2026. The class includes Medicare-eligible retired city workers and dependents enrolled in the medical GHI Senior Care Plan January 1, 2022 through January 31, 2023.

The administrator says notices go to all class members, but personalized claim forms only go to those records identify as potentially exposed to eligible $15 copays. A notice without a form does not establish a right to reimbursement, even though the person may remain bound by the settlement if approved.

Email is not a valid claim route

Eligible recipients must complete and submit the claim by mail or online. The administrator explicitly says emailed claims are not accepted. Receiving or returning a form does not make reimbursement automatic; eligibility and processing still apply.

Exclusion is due October 15 and objections November 30 under the separate notice instructions. A later objection deadline does not extend the October claim window. Keep the notice, actual copay records and submission confirmation rather than assume every retiree receives a share of a headline fund or that settlement approval has already occurred.
