The claim deadline is not the loss cutoff

Inotiv data-breach settlement claims are due December 2, 2026. The notice concerns the August 2025 incident and offers two years of medical-identity protection plus cash choices. The company denies wrongdoing, and class eligibility is not established merely by knowing its name.

The documented route includes ordinary out-of-pocket expenses up to $500, extraordinary fraud or identity-theft losses up to $4,000 and actual response time up to four hours at $20 per hour. The loss periods stated in the notice run August 5, 2025 through September 10, 2026, earlier than the claim deadline.

Separate loss categories need separate support

Alternatively, class members may request $45 instead of other cash payments without loss proof or explanation. Already reimbursed losses cannot be claimed again. Personal notes can clarify evidence but do not replace the third-party support needed for a documented loss.

Exclusion and objection are due November 2, with the final hearing scheduled December 3. Keep the notice login information and use the administrator instructions. A maximum sum of multiple categories is not a flat award; each category has its own conditions, and no individual payment has been verified here.
