The class list and notice matter

The class concerns people on the settlement list whose private information was potentially compromised in the December 2024 incident, not every person who has received medical care in Fort Wayne.

The notice distinguishes ordinary documented expenses up to $500, extraordinary qualifying losses up to $5,000 and limited lost-time reimbursement. Lost time counts toward the ordinary-expense cap. Supporting proof and a connection to the incident are required for the relevant loss routes.
