The notice identifies affected information

The Eisner Advisory Group data-settlement claim form lists October 9, 2026 for online claims or mailing postmarks. It concerns people notified that information was potentially subject to unauthorized access in an incident involving EisnerAmper and Eisner Advisory Group. It is not a rebate for all accounting clients.

The $1.05 million fund covers class relief and approved fees, administration and service awards. The form estimates a proportional cash payment of $100, which can rise or fall. It also allows fairly traceable documented monetary losses up to $5,000, requiring reasonable third-party evidence.

One headline number does not describe every claim

California subclass members have a separate $50 route for the CCPA claim, subject to reduction. Two years of monitoring can also be requested. The form permits claims for one or more benefits under its terms, so it differs from settlements that force a single flat-cash-versus-loss choice.

Self-prepared receipts do not establish documented losses, though explanations may support official records. Use the actual form selections and class definition rather than assuming a current California address is enough. Final approval is scheduled October 13; the claim form is a request, not confirmation that funds have been paid.
