This settlement allows more than one cash category

The DAP Health data-settlement website lists October 21, 2026 for online claims or mailing postmarks. The $1.3 million proposal concerns private information compromised in its July 2024 incident. Start with the affected-person notice and class definition, not a healthcare-provider name alone.

Unlike settlements that make cash choices exclusive, this administrator lists documented losses up to $5,000 and a proportional cash payment estimated at $25. California residents on July 22, 2024 may claim an additional $75 statutory payment. These categories have separate conditions.

The passed hearing date is not an approval finding

Two years of credit and medical-record monitoring may also be claimed. The $25 is an estimate, not a guaranteed check, and loss reimbursement requires the notice evidence conditions. A possible stacked benefit does not mean every claimant has incurred a maximum reimbursable loss.

Exclusion and objections closed September 1. The site still lists an October 1 final hearing and preliminary-approval language; this check did not independently establish the hearing outcome. Claims remain separately listed through October 21. The company disputes wrongdoing. Follow the current form and keep confirmation instead of assuming the passed hearing means payment has begun.
