Similar provider names do not establish eligibility
The Community Dental Care data settlement lists October 19, 2026 for online claims or mailed postmarks. It concerns a December 2024 intrusion involving private and health information. It is not the similarly named Community Connections behavioral-health case.
The cash choice is actual unreimbursed documented losses up to $5,000 or alternative $50 cash. The FAQ explicitly prohibits claiming both. Losses must be caused by the incident and fall within the specified December 20, 2024 through October 19, 2026 period.
Loss payments and alternative cash are different choices
Two years of medical and credit monitoring can accompany the cash choice. A $1 million monitoring insurance figure is not an award to every patient. Keep breach notices and official records, and avoid claiming expenses already paid by an insurer or another source.
Exclusion and objections closed September 24, while final approval is scheduled November 23. The company disputes the claims, and benefits depend on approval and appeals. A treatment history alone is not proof the relevant information was affected or that a payment is guaranteed.
