The notice covers two incidents

The Community Connections data-settlement website lists October 6, 2026 as the cash-claim deadline. It concerns incidents discovered in October 2024 and April 2026 at the behavioral-health provider. Eligible people include those whose private information was impacted and those sent notice letters.

The administrator distinguishes documented-loss cash, lost-time cash and alternative cash. The alternative replaces the first two routes rather than automatically stacking on top of them. Follow the actual claim selections and evidence conditions, not a headline that adds every available option together.

Doing nothing does not produce cash

A valid claim is required for cash. Members who do nothing receive a monitoring enrollment code but no cash payment, while remaining bound by the settlement if approved. Receiving a code is not proof a monitoring service has already been activated.

Exclusion and objection deadlines passed September 21. The company denies the allegations about failing to prevent the incidents. Keep the incident notice and relevant records, use the official administrator and avoid interpreting a settlement deadline as evidence every patient is in the class or every claimant is paid today.
